Where NHS and Council Support Ends in Somerset & Avon — We Begin
This region is unusual, and it is worth explaining properly because almost nobody does. Somerset and Avon is not one NHS area. It is two, and which one you live in changes everything.
In the north — Bath, Keynsham, Radstock and the villages around them — community health is delivered by HCRG Care Group on behalf of the NHS, and now runs under the name BSW Community Health. Their adult physiotherapy and musculoskeletal service is good and easy to reach: you can refer yourself without a GP, and the line is open seven days a week, 8am to 8pm. But it is a clinic service. In this area their sites are Keynsham Health Centre, Paulton Memorial Hospital and St Martins Hospital in Bath. There is no home visiting option listed. Hospital care comes from the Royal United Hospitals Bath NHS Foundation Trust.
In the south — Yeovil, Wincanton, Castle Cary and Shepton Mallet — you are in a completely different system. Hospitals and community services are run by Somerset NHS Foundation Trust, which has thirteen community hospitals, seven urgent treatment centres and a full emergency department at Yeovil. Support after a hospital stay comes through Somerset Council's intermediate care and Discharge to Assess service, part of its Home First approach — free, including occupational therapists, and capped at six weeks.
⚠️ And if you are in South Bristol, it is different again. That falls under a third integrated care board, with its own community provider and its own hospitals. If you are unsure which applies to you, call us and we will tell you what we know rather than guess.
Whichever half of the region you are in, though, the ending is the same. If you can get to a clinic, go — it is free and, in the north, you can refer yourself today. If you cannot get to a clinic, or your six weeks of reablement have run out, the exercises are still yours to do alone in your own house.
That is the gap we fill. We come to the house — after reablement ends, or from the start if getting to a clinic was never realistic. We keep going for as long as it is genuinely helping, and we work alongside NHS care rather than instead of it. No waiting list, no GP referral.
