Where NHS rehabilitation ends, and how we pick up from there.
Bradley Stoke and Stoke Gifford have a major hospital on the doorstep, and it would be dishonest to open any other way. Southmead Hospital, the region's major trauma centre, is just south of Filton, run by Bristol NHS Foundation Trust, which was formed in July 2026 from the city's two hospital trusts. Planned hip and knee replacements are done at the Avon Orthopaedic Centre at Southmead, the largest orthopaedic centre in the region. For urgent problems that are not emergencies, the minor injuries unit at the Yate West Gate Centre, run by Sirona care & health, is open from 8am to 8pm every day, with X-ray.
So the honest question is: with all of that nearby, why would you need us?
Because the hospital is where the operation happens, and home is where the recovery happens. A hip replacement, a knee, a fractured wrist or hip — the surgery takes a morning. The rebuilding takes weeks, and almost all of it happens in your own house. A hospital on the doorstep does nothing for you at week three, when you still cannot manage your own stairs and the exercise sheet is on the kitchen table.
Outpatient physiotherapy is the other half of it. It exists, and it is good, but it means getting there — down to Southmead — and parking, on a leg or a hip that is not ready for any of that. For a lot of older people the journey is harder than the treatment, and appointments get missed for exactly that reason.
NHS therapy at home comes through Sirona care & health, whose community teams include physiotherapists and occupational therapists. It is for people who can't get to outpatient appointments, and it is arranged through a referral from a health professional, such as your GP or the hospital team. Worth knowing: Bristol's two hospital trusts merged on 1 July 2026, so letters and websites may still say North Bristol NHS Trust or University Hospitals Bristol and Weston. They are the same hospitals.
Where there is council support, it comes through South Gloucestershire Council's reablement service: short-term support at home to help you get your independence back after a hospital stay, free for up to six weeks. We continue the work after that, or from the start if reablement was never part of the picture, for as long as it's genuinely helping and alongside anything the NHS already has in place.

Why would I need a home physiotherapist when Southmead is on the doorstep?
Because the hospital is where the treatment starts, and home is where the recovery happens. Southmead Hospital has a 24-hour emergency department and the Avon Orthopaedic Centre, and there is a minor injuries unit in Yate. They are good services. But a hip replacement takes a morning to do and weeks to rebuild — almost all of it in your own house. Getting to an outpatient appointment and parking is, for many older people, harder than the treatment itself. We come to you instead, and build your programme around your actual stairs, your actual chair and your actual bathroom.

So a lot of our work here is with people of working age — after a knee operation, a shoulder repair or a back injury — as well as with older people after a fall or a hospital stay.
That is who a home visit is for. A fall at home, a new hip, a spell in hospital that leaves someone weaker than when they went in — and a family, often living some distance away, trying to arrange help quickly.
And running through all of it is the same point. A hospital on the doorstep does nothing for you at week three of a hip replacement, when the exercises are on the kitchen table and nobody has watched you try your own stairs. That is the bit we do.
