Where NHS and Council Support Ends in Wiltshire — We Begin
Let us be straight about the NHS option first, because it is better than a lot of people realise and you should use it if you can.
Community health services across Bath and North East Somerset, Swindon and Wiltshire have been delivered by HCRG Care Group on behalf of the NHS since April 2025, and now operate as BSW Community Health. Their adult physiotherapy and musculoskeletal service takes self-referrals — you do not need to go through your GP — and the line is open seven days a week, 8am to 8pm, on 0300 247 0200. For back, neck and joint pain, sports injuries and post-surgery rehabilitation, that is a good service and it is free.
But it is a clinic service. Their own page lists twenty-one locations across the three counties — health centres, community hospitals, even a leisure centre. In Wiltshire those include Salisbury District Hospital, Five Rivers Leisure Centre, Amesbury Health Centre, Trowbridge Integrated Care Centre, Wilton, Warminster, Melksham, Devizes, Chippenham, Calne, Malmesbury, Savernake and Westbury. There is no home visiting option listed anywhere on it.
And there are gaps. Corsham, Downton and Bradford-on-Avon have no clinic of their own. If you live in one of those, you are travelling to Chippenham, Wilton or Melksham — which is fine if you drive, and a very long morning if you do not.
So here is the honest version. If you can get to a clinic, go. It costs nothing, you can refer yourself, and the staff are good. If you cannot get to a clinic, that is the entire reason we exist.
The same applies after a hospital stay. Wiltshire Council's reablement support is free and genuinely useful, but it is time-limited — typically around six weeks. When it ends, the exercises are still yours to do, alone, in your own house. And which hospital you deal with depends on where you live: Salisbury NHS Foundation Trust in the south, Great Western Hospitals NHS Foundation Trust in Swindon to the north, and the Royal United Hospitals Bath NHS Foundation Trust for Trowbridge, Corsham and Bradford-on-Avon in the west.
We come to the house. After reablement ends, or from the start if getting to a clinic was never realistic. We keep going for as long as it is genuinely helping, and we work alongside NHS care rather than instead of it. No waiting list, no GP referral.
