Neither Castle Cary nor Bruton has a hospital. Somerset NHS Foundation Trust runs thirteen community hospitals across the county — Bridgwater, Burnham-on-Sea, Chard, Crewkerne, Dene Barton, Frome, Minehead, Shepton Mallet, South Petherton, Wellington, West Mendip, Williton and Wincanton. Neither of our towns is on that list.

The two nearest are Shepton Mallet and Wincanton, each six or seven miles away. Shepton Mallet is the more useful of the two if something needs seeing quickly: it has an urgent treatment centre, a medical assessment unit, an out-of-hours GP service and a falls and frailty clinic. Wincanton has inpatient beds, outpatient clinics, MSK physiotherapy and X-ray, but no urgent care.

Opening hours at the urgent treatment centres do vary, and they can close early when they reach capacity, so it is worth checking 111 online before setting off rather than driving over and finding the door shut.

For an emergency, the nearest A&E is Yeovil District Hospital, about a dozen miles to the south, which has a 24-hour emergency department.

The practical consequence is the same as everywhere else out here. Your operation happens in Yeovil, and then you come home. A hip replacement, a knee, a shoulder repair, a fracture after a fall — the surgery takes a morning. The rebuilding takes weeks, and it happens in your own house whether anyone has arranged for it or not. Outpatient physiotherapy is available at Wincanton and Shepton Mallet, but it still means getting there and back, repeatedly, on a leg that is not ready for it.

Where there is council support it comes through Somerset Council's intermediate care and Discharge to Assess service, part of its Home First approach. It is free, it includes occupational therapists, and it is capped at six weeks. We continue the work after that, or from the start if intermediate care was never part of the picture, for as long as it's genuinely helping and alongside anything the NHS already has in place.

Castle Cary is genuinely an old town. Close to a third of its residents are aged 65 or over, against 18.4 per cent for England. In a town of a few thousand people, that shapes almost everything: how often somebody falls, how long they take to get going again, and how many households are quietly managing on their own.

Bruton looks younger on paper, and we should explain why rather than pretend otherwise. King's Bruton, Sexey's School and Bruton School for Girls all take boarders, so roughly one in five people counted as living in Bruton is a schoolchild aged between ten and nineteen. Strip the schools out and the resident population looks a great deal more like Castle Cary's. The town feels older than its statistics, and that is the honest reading of it.

Neither town has a hospital. The nearest are Shepton Mallet and Wincanton, six or seven miles off, and the nearest A&E is a dozen miles away in Yeovil. For someone driving, that is a nuisance. For someone three weeks out of hospital, or frightened of falling again, it is the reason treatment quietly stops happening.

And the villages make it harder still. Yarlington, Shepton Montague, Ditcheat, Upton Noble, the Brewhams — small places on narrow lanes with barely a bus between them. A hospital appointment is a planned expedition, not an errand.
