Let's start with the plain fact. There is no hospital in Somerton, and none in Langport. No inpatient beds, no minor injuries unit, no urgent treatment centre. Both towns have a GP surgery and a pharmacy, and beyond that everything happens somewhere else.

Somerset NHS Foundation Trust runs seven urgent treatment centres — at Frome, Glastonbury, Shepton Mallet, Chard, Bridgwater, Minehead and Yeovil — plus one minor injuries unit at Burnham-on-Sea. The nearest to us are West Mendip at Glastonbury and Yeovil. Neither is on the doorstep, and opening hours do vary, so it is worth checking 111 online before setting off rather than driving over and finding it closed.

For an emergency there are two A&E departments in range, and this area sits awkwardly between them. Yeovil District Hospital is to the south-east and Musgrove Park Hospital in Taunton is to the west, both run by Somerset NHS Foundation Trust. Which one is quicker depends on where you live and which way the roads are running.

The practical consequence is straightforward. Your operation happens in Yeovil or Taunton, and then you come home to a village. A hip replacement, a knee, a shoulder repair, a fracture after a fall — the surgery is the short part. The rebuilding takes weeks, and it happens at home whether anyone has arranged for it or not. Outpatient physiotherapy exists, but it means getting to Yeovil or Taunton and back, repeatedly, on a leg that is not ready for it.

Where there is council support it comes through Somerset Council's intermediate care and Discharge to Assess service, part of its Home First approach. It is free, it includes occupational therapists, and it is capped at six weeks. We continue the work after that, or from the start if intermediate care was never part of the picture, for as long as it's genuinely helping and alongside anything the NHS already has in place.

Nearly a third of Somerton is aged 65 or over, and Langport is barely behind it. Against a national figure of 18.4 per cent, that is not a small difference. In towns of a few thousand people, it changes what a fall means, how long a recovery takes, and how many households are quietly managing on their own.

And there is no hospital here. Not a reduced one, not one with fewer beds than it used to have — none at all. Two GP surgeries, two pharmacies, and then Glastonbury, Yeovil or Taunton for anything else.

The Levels make that harder rather than easier. This is flat, open, sparsely populated country where villages sit a few miles apart along narrow roads, bus services are thin, and winter weather can make journeys slower still. Getting to an outpatient appointment in Yeovil or Taunton and back is most of a day for someone fit. For someone three weeks out of hospital, it often simply does not happen, and the recovery quietly stalls.
