Chard is the best-provided town in this part of Somerset for urgent care, and we should say so plainly before anything else. Chard Community Hospital on Crewkerne Road has an urgent treatment centre, open seven days a week from 8am to 9pm. It also runs a long list of outpatient clinics — care of the older person, musculoskeletal, pain management, rheumatology, respiratory and more. Most towns this size have nothing like it.

Two things are worth knowing all the same. X-ray runs on Mondays, Tuesdays and Thursdays only, from 9am to 5pm. If you turn up with a suspected fracture on a Wednesday or at the weekend, you will be assessed and treated, then asked to come back another day for the X-ray — or sent on to an acute hospital if it cannot wait.

And the urgent treatment centre can close at short notice. Somerset NHS Foundation Trust says so itself: units may shut to new patients before their normal closing time when they reach capacity, and occasionally close for a whole day. It is always worth checking 111 online before setting off.

Chard is also the one town round here whose A&E is not Yeovil. The nearest emergency department is Musgrove Park Hospital in Taunton, about fourteen miles west — a different hospital, in a different direction, from the one that serves Crewkerne, Wincanton and Castle Cary.

The rest of the story is the same as everywhere. Your operation happens in Taunton, and then you come home. A hip replacement, a knee, a shoulder repair, a fracture after a fall — the surgery is the short part. The rebuilding takes weeks, and it happens in your own house whether anyone has arranged for it or not.

Where there is council support it comes through Somerset Council's intermediate care and Discharge to Assess service, part of its Home First approach. It is free, it includes occupational therapists, and it is capped at six weeks. We continue the work after that, or from the start if intermediate care was never part of the picture, for as long as it's genuinely helping and alongside anything the NHS already has in place.

Chard does better than its neighbours, and it would be dishonest to open any other way. It has an urgent treatment centre, open seven days a week until nine in the evening, and a proper list of outpatient clinics. Somerton and Langport have nothing. Castle Cary and Bruton have nothing. Chard has both.

But an urgent treatment centre is for the day something goes wrong. It is not for the six weeks afterwards, and that is the part we deal with.

Chard is also an older town than the country as a whole — around a quarter of residents are over 65, and in Ilminster it is closer to three in ten. In practice that means falls, and hips, and knees, and the long slow business of getting steady again once the ward has discharged you.

The geography does not help. Chard sits at the very bottom of Somerset, on the Devon border, and it is the one town round here whose emergency department is Musgrove Park in Taunton rather than Yeovil — fourteen miles west, in the opposite direction to everywhere else in the county. Outpatient physiotherapy means that journey, or the one to Chard hospital, over and over, on a leg that is not ready for it.

And the villages are further still. Winsham, Buckland St Mary, Whitestaunton, Dowlish Wake — small places on narrow lanes with barely a bus between them. For someone three weeks out of hospital, the appointment quietly stops happening and the recovery quietly stalls.

So we come to the house. We look at the actual stairs, the actual chair, the step down into the kitchen, and we work on what is genuinely stopping you — for as long as it's helping, and alongside whatever NHS or council support you already have.
