Where NHS rehabilitation ends, and how we pick up from there.
Downton has a major hospital a few miles up the road, and it would be dishonest to open any other way. The nearest emergency department is at Salisbury District Hospital on Odstock Road, run by Salisbury NHS Foundation Trust as part of the BSW Hospitals Group, open 24 hours a day. It carries out hip and knee replacements, and it is home to the Duke of Cornwall Spinal Treatment Centre.
So the honest question is: with all of that within reach, why would you need us?
Because the hospital is where the operation happens, and home is where the recovery happens. A hip replacement, a knee, a fractured wrist or hip — the surgery takes a morning. The rebuilding takes weeks, and almost all of it happens in your own house. A hospital at Odstock does nothing for you at week three, when you still cannot manage your own stairs and the exercise sheet is on the kitchen table.
Outpatient physiotherapy is the other half of it. It exists, and it is good, but it means getting there — up the road to Odstock — and parking, on a leg or a hip that is not ready for any of that. For a lot of older people the journey is harder than the treatment, and appointments get missed for exactly that reason.
NHS community health services across Wiltshire are run by HCRG Care Group under the name BSW Community Health. Some of its services take self-referrals; others need a referral from your GP or another health professional. Worth knowing: until April 2025 these services were run by Wiltshire Health and Care, so you may still see that name on older letters, leaflets and websites.
Where there is council support, it comes through Wiltshire Council's Reablement Service. Its teams include occupational therapists, and it lasts up to six weeks. We continue the work after that, or from the start if reablement was never part of the picture, for as long as it's genuinely helping and alongside anything the NHS already has in place.

Why would I need a home physiotherapist when Salisbury District Hospital is so close?
Because the hospital is where the treatment starts, and home is where the recovery happens. Salisbury District Hospital has a 24-hour emergency department and carries out planned hip and knee replacements, and it is a good hospital. But a hip replacement takes a morning to do and weeks to rebuild — almost all of it in your own house. Getting to an outpatient appointment and parking is, for many older people, harder than the treatment itself. We come to you instead, and build your programme around your actual stairs, your actual chair and your actual bathroom.
How much does a home physiotherapy visit cost in Downton?

Downton has one of the region's major hospitals a few miles away at Odstock. That is fortunate, and it is worth saying first.
It is also older than England as a whole. In the Downton & Ebble Valley ward, 28.1 per cent of people are aged 65 or over, against 18.4 per cent for England, and across Wiltshire the figure is 21.9 per cent. Downton lost its railway station in 1964, so hospital appointments mean a car, a bus or a lift.
